HLA-B27 Antigen

General aspects

HLA (Human Leukocyte Antigen) histocompatibility system is constituted by a
set of molecules placed on the human cells surface stimulating an immune response if recognized as foreign.

HLA system has a foundamental role in graft’s rejection; when receiving a tissue graft, the recipient's immune system will react to and attack the donor tissue unless it expresses the same set of tissue antigens as the recipient's, for which it has learned not to react against.
Through not completely clear mechanism yet, some antingens are able to stimulate an immune response against substances and tissues normally present in the body (autoimmune diseases) leading anatomical and/or functional damage.
Several studies show an association between the HLA-B27 presence and some autoimmune diseases onset, such as spondyloarthritis (ankylosing spondylitis (Fig.1), Reiter syndrome, psoriatic spondyloarthritis ...) and acute anterior uveitis (eye’s inflammation); for example ankylosing spondylitis patients (80-90%) have HLA-B27antigen. In general population the that frequency is variable and it is highest among the Caucasians people (8%).

HLA-B27 DNA test may have a double value, from a side it fits into "preventive medicine" context: knowing an antigen carrier state, that it doesn’t means a disease development, suggests an increased risk and it can be functional in order to take more suitable measures to prevent disease onset. In affected patients, instead, HLA-B27 DNA test could be useful to determine if the pathology is or not HLA-B27 antigen’s presence associated.
Genetic counseling
